Sinopsis
Veinte años después de la última entrega, John Rambo (Sylvester Stallone) se ha retirado al norte de Tailandia donde trabaja en una lancha por el Río Salween. En la cercana frontera entre Tailandia y Birmania, en Myanmar, el conflicto entre birmanos y karennis, la guerra civil más larga en toda la historia del mundo, lleva durando ya 60 años. Sin embargo, Rambo, quien lleva una vida solitaria en la jungla que cubre las montañas, pescando y cazando serpientes venenosas que luego vende, hace tiempo que ha abandonado el combate, aunque por su lado pasan médicos, rebeldes y refugiados camino de las tierras asoladas por la guerra. Todo eso cambia cuando un grupo de misioneros está buscando al “guía americano del río”. Cuando Sarah (Julie Benz) y Michael Bennett (Paul Schulze) se acercan a Rambo, le explican que desde la expedición del año pasado a los campos de refugiados, los militares birmanos han diseminado minas de tierra a lo largo de los senderos, haciéndolos extremadamente peligrosos para los desplazamientos terrestres. Le piden que les guíe Río Salween arriba y les deje donde puedan entregar suministros médicos, alimentos y biblias a la gente karenni perseguida. Pese a negarse inicialmente a penetrar en Birmania, Rambo acaba aceptando llevarles y dejarles, a Sarah, Michael y otros colegas de la misión de auxilio, en un lugar determinado.
